KASHKALOGLU EYE HOSPITAL APPLICATION PROCEDURE FOR DATA SUBJECT PERSONAL DATA
GENERAL EXPLANATIONS:
In the Law No. 6698 on the Protection of Personal Data (“KVKK”), personal data owners (“Data Owner”), who are defined as “relevant person” in the Law on the Protection of Personal Data No. 6698 (“KVKK”), are entitled to apply to the Data Controller Laser Myopia Special Treatment Center Limited Company (“Kaşkaloğlu Eye Hospital“) as the Data Controller regarding the rights listed in Article 11 of the KVKK regarding the processing of personal data. The applications to be made to our Company regarding these rights must be submitted to our Company in writing or electronically and by the following methods in accordance with paragraph 1 of Article 13 of the KVKK and the relevant provisions of the Communiqué on the Procedures and Principles of Application to the Data Controller.
APPLICATION METHODS:
Pursuant to Article 5/2 of the Communiqué on the Procedures and Principles of Application to the Data Controller, which entered into force after being published in the Official Gazette dated 10.03.2018 and numbered 30356, in the applications to be made to our company within the scope of the Law on the Protection of Personal Data, it is obligatory to include the name, surname and signature if the application is in writing, T.R. identification number for citizens of the Republic of Turkey, nationality, passport number or identification number, if any, for foreigners, residential or workplace address for notification, e-mail address for notification, telephone and fax number and subject of the request.
Written Applications:
Written applications to be made to our Company must be accompanied by a wet signed copy of this “Application Form” containing the above information and filled in completely,
- It should be delivered to our Company in person or by proxy with a notarized power of attorney indicating that it is authorized to apply for the rights listed in Article 11 of the KVKK or by sending it to “Mimar Sinan Mahallesi 1400 Sokak No: 10/Z1 Konak İzmir“ address through a notary public.
Electronic Applications:
Electronic applications to be made to our Company shall be made by submitting this “Application Form” containing the above information and filled in completely.
- By signing with an electronic or mobile signature with a “secure electronic signature” certificate defined in the Electronic Signature Law No. 5070 and sending it to lasermiyopiozel@hs03.kep.tr, which is the Registered Electronic Mail (KEP) address of our Company,
- By sending it to info@kaskaloglu.com via the e-mail address belonging to the relevant person making the application and previously notified to our Company by this relevant person and registered in our Company’s system,
Depending on the nature of your request, your applications submitted to us will be finalized in an effective, lawful and honest manner “as soon as possible and within thirty days at the latest” from the date your request is received by our company and will be answered in writing or electronically.
In the event that any deficiency is detected by our company regarding your application, if the deficiency is not corrected by you within seven (7) days at the latest from the notification of this deficiency to you, your request will be suspended until the deficiency is corrected by our company.
However, if the transaction requires an additional cost, pursuant to Article 7 of the Communiqué on the Procedures and Principles of Application to the Data Controller, no fee will be charged up to ten pages, and a transaction fee of 1.00 Turkish Liras will be charged for each page over ten pages.
It is important that the content of your request is clear, understandable and dated. For this reason, the information and documents requested by you with this application form must be filled in completely and accurately and applied to our company in the above ways.
